Description
All organization interested in being formally acknowledged with a Five Star credential must complete the fallowing steps (The complete process is online):
1.Have completed the Self-Assessment Process.
2.Have obtained 225 points or above the Likert point scale.
3.Completed and submit the Application.
4.Pay the Five Star Certification Fee.
5.Upon approval of the application, submit evidence of documentation on 30 indicators that will be randomly selected by our professional staff.
6.Validation of points on indicators.
7.Official letter of Five Star Acknowledgment or denial.
8.Five Stars Certification → Sample
9.Organization listed on Five Star Certification Credentials.
10.Five Star Certification valid for period of five years.
